What is ideal for Tweets Privacy. Should it be visible to all or secured for only the followers

It really depends on what you're using the Twitter account for. If it's a general page for your hospital/organization then it would make much more sense to keep it visible to all. I can't think of many scenarios where it would make much sense to secure your tweets.

I believe if you want to keep your private/personal profile to yourself, that is more than okay to make it private. There is a battle between wanting to be your own brand as someone in communications/media and also wanting to keep your personal life, personal. That really has to be a personal choice.
